Dear Parent,

The most common reasons an application is rejected at the DMV; missing even one item causes the application to be denied and makes a wasted trip

  1. Didn't bring the white Certificate of Enrollment for Driver Training OL 392 (proof of enrollment for the six hours behind the wheel)
  2. Didn't write the Social Security  Number on the DL 44 application form
  3. Tried to use a photo copy of the birth certificate (An original or certified copy of the birth certificate is required)
  4. Forgot the Blue (or pink) Driver Education Completion Certificate OL 237, (the proof of completion of the 30 hour classroom)
  5. The DMV application form DL 44 was not signed by both parents

DMV requires your teenager to take at least one lesson with a professional instructor in a training car with an instructor's brake before going with a parent
  11. After the permit is validated by having completed the first two hour lesson, the student may legally drive with a parent in an isolated quiet area the first few times.
